All fees must be received with registration. Registrations must be received before the dates noted to receive special rate. Any registration received after August 20, 2009 will be considered On-Site and charged an additional $30 per registrant.

Special Workshop hotel rates: Standard rate $179 single/double and suite rate $273 single/double. Registrants should make hotel arrangements for the Workshop by completing and faxing or mailing the enclosed reservation form to The Pfister Hotel as early as possible. No call-in reservations will be accepted. Cancellations or changes made within 21 days of arrival will result in full payment of room and tax for every room night cancelled.

All cancellations must be received in writing. There will be a charge of $50 per person for cancellations received after July 31. No refunds for cancellations received after August 20. Interest charges of one and one half percent per month will be added to delinquent accounts, together with any attorney’s fees and cost of collection if necessary.
