• Member Companies that engage in predominately Crane Rental or Contracting and completed one full year of SC&RA membership.

You’ll need the following information:

1. Total Number of Man Hours
2. Total Number of Recordables
3. SC&RA's Work-Related Injuries and Illness Log (found on SC&RA Awards Webpage)
4. Insurance information:

U.S. REQUIREMENTS (Please Attach All):
• EMR on insurance broker/agent’s letterhead.

CANADIAN REQUIREMENTS (Please Attach All):
• Workers Compensation Board Injury Reporting form.*
• Workers Compensation Board Premium Rate Report.*
*For all provinces where the company does business.

• The Company nominating the individual operator must be a current member of SC&RA and have completed one year of company membership in the Association.

You’ll need the following information:

1. Each operators total number of operating hours (must be over 10,000)
2. An Individualized letter of recommendation signed by the member company’s safety director on your company’s letterhead providing additional pertinent information regarding the crane operator you are nominating for this award.
3. Each individual operators nationally accredited certification(s) and drug test results
4. Any incidents relating to the individual operator(s) and related incident cost(s)

• The Company nominating the individual rigger(s) must be a current member of SC&RA and have completed one year of company membership in the Association.

You’ll need the following information:

1. Each riggers total number of operating hours (must be over 10,000)
2. An Individualized letter of recommendation signed by the member company’s safety director on your company’s letterhead providing additional pertinent information regarding the rigger you are nominating for this award.
3. Each individual rigger nationally accredited certification(s) and drug test results
4. Any incidents relating to the individual rigger(s) and related incident cost(s)
